The City has a unique opportunity for an experienced, talented and passionate individual to act as a Coordinated Access Specialist to work alongside community agencies under the Reaching Home Program to design and implement the City’s Coordinated Access and Assessment (CAA) System, ideally starting October 2022 for an 18-month period, with the potential for longer.
The Coordinated Access Specialist, reports to the Housing, Homelessness and Community Development Specialist, and works collaboratively with key partners internally and in the community including senior government, health service delivery, the non-profit sector and people with lived experience of homelessness to prepare and develop a CAA System in Abbotsford. In developing the CAA System, the Coordinated Access Specialist will be responsible for bringing all major service providers directly serving people who are homeless together to develop a shared barrier free service access process including common screening and referral tools, shared information management system (HIFIS database), and other supports that increase collaboration and convenient service access in Abbotsford.
To support the development and implementation of the CAA system, this position will include working with stakeholders on the development and evaluation of supporting policies, plans, protocols, and processes pertaining to the System.. Part of this work will include developing a ‘By Name List’, to support timely coordination and information sharing, as well as establishing and facilitating a ‘Situation Table’.
As the ideal candidate you will have:

  • A undergraduate degree in social or health planning, or related discipline and 5 to 7 years of related experience.
  • Experience should include municipal, regional or provincial government social and health planning, related research, public engagement, project management and policy writing.
  • High level of knowledge of and strong interest in social policy and programs related to homelessness response, housing delivery, health and social services and non-profit delivery systems.
  • Ability to hold a high level of confidentiality with community service and client files and histories.
  • Ability to conduct comprehensive community-based stakeholder engagement and research as well as write concise, well-written policy reports regarding homelessness, housing and health/social support service access and systems coordination to support assessment, access, delivery and monitoring of housing and services to address homelessness.
  • Some understanding of information management systems or databases for case planning and service delivery.
  • An understanding of the various personal, systemic, structural and cultural factors that contribute to homelessness, mental health and substance use issues within human populations.
  • Excellent project management skills, including the ability to plan, organize and deliver projects and assignments, and ensure successful implementation (on time and on a budget).

This position requires an acceptable criminal record check and signing of confidentiality agreements.
